Say this 3 times while looking at your animal: "Spirit of the (animal) please help grant this wish:
I wish to be a/n (animal) like you with eyes of (colour) and (colour) fur/feather, I have full control and shift whenever I want. I will have powers of (power) in my (animal) form. Spirit of the (animal) please, grant me this wish so mote it be."

Instead of say I wish to be, could I say I wish to have, and then it would summon the animal I want?

I guess that would work too. There are spells for summoning the animal you want [Obviously they do not work, and neither does this spell].

Ik that is highly unlikely, but it doesn’t hurt to ask

You can't become an animal, but you could connect with an animal spirit [this spell specifically, probably won't do anything though]

Can I use a real animal, like my pet?

You can't physically transform into an animal, but you can work with your pet. You could connect with its spiritual energy and mentally or spiritually shift as a result of your spiritual connection with them. If you want to try this spell with your pet, give it a go and see what happends.
