The Grim Reaper aka Death, is the personification of death in the form of a cloaked skeleton holding a large scythe, that thing with the long, curved blade, that used to be used to cut grass many many years back. Go to this site for a little info:

Grim Reaper is a concept of death.In more path we can find a lot of concepts of death (Angel of Death for example is another concept of death)
