Typically, unless you are wanting to work with the spirit of the one in the grave, it is generally taken from the graveyard itself, rather than a specific grave. It doesn't need to be a fresh grave, if you want to work with the spirit of the person residing in the grave, best to spend several weeks, even months forming a bond with the spirit before taking their dirt for workings.

Yes. Graveyard dirt has to be from a old grave snake skin and sulfur coffin nails
