Gather bones of chickens and dry them in the sun for a few days. Then when you are ready to do this hex make sure you are worked up into a frenzy of anger and hatred. This will add to the potency of your hex! Be thinking of all this while doing this hex and when it says:
'With these bones I now do crush."
Take a hammer or use your feet to stomp and crush these bones as if they were your enemy before you! When you are done sweep them up and place them in a bag. You will then want to sprinkle the dust and remains of the bones on your enemies property around his house.
If you have a bell ring it 3 times and say:

This curse shall be laid upon the victim while burning am image of the
victim (wax sigil, photograph, drawing, anything) in the flame of a
consecrated black candle.
Light two black candles on the alter you have worked up the negative energy and your anger has built... take the Voodoo doll and visualize with intensity that it is your enemy before you. You may even cut out the face on a picture of your victim and glue it to the head of the Voodoo doll. If you have any nail clippings or hair follicles or any small item that has the victims vibration on it, you can place it inside the Voodoo doll. That will make it a much stronger connection and all the more powerful. Once you have worked up the negative energy and your anger has drawn near its boiling point... slam it to the alter and stab one time at the doll in the place where you wish to afflict pain. Then take another pin if you so desire and prick the doll once again in a place where you wish to afflict more pain. Each time you stab at the Voodoo doll make sure you are visualizing that you are actually inflicting pain on your enemy. Do not touch the pins after you have placed them. While doing this, recite the following incantation....

You may repeat this again for a few days if you wish... adding more pins and leaving the pins that are already there alone. Do not touch the pins after you have placed them. Once again, if you touch any of the pins after they have been placed, the Curse will come back on you. Backfire!

First, get to a place outside with all the ingredients. Make sure no one can disturb you or interrupt you during this ritual otherwise it will not work.
If you want to make this do a death spell, instructions is further down below the first chant. Then you draw a circle in the ground, here's how to do it: Take up a stick to draw the circle on the ground, make sure it's even, a full circle. Then place the candle (a big one) in the middle of the circle and light it. With a concentrated mind and focused voice, chant this:
"Messenger of Darkness, hear my call, messenger of darkness, come to me. Messenger of darkness, hunt thee. (Name) ______, for the harm he/she has done, the crime shall be paid in time." Chant this three times while burning the black napkin in the fire.
When the napkin is fully burned up, snuff out the candle (don't blow cause it can disturb the energy and give a reversed effect, you might be the victim for this dark spell) then clean up after yourself and wait.
Making it to a death spell:

Before you cast the spell, bury the cloth in the ground for 3 days. Dig it up and you're ready to cast the rest of the spell.
Lay out the playing card, then a piece of paper with the other person's name on it, then cover with the cloth. Drive a nail through the center of all 3 items (have something sturdy underneath, like a piece of wood). Say out loud the words of the spell:
By the power of my will,

Bury the whole charm where you had the cloth at the start. Leave it for 3 more days, then dig it up again. Burn it, and the spell is complete.

Odds of having a rotten egg just sitting around are pretty slim, so you should plan on doing this spell in advance. Leave a whole (uncracked) raw egg out in the sun for several days until is starts to smell a little ripe. It can take a week or even more.
When you are ready to do this spell (try for a Saturday), remember that it will smell bad and you might not want other people home when you do it.
Set out the photo on a table, on a plate or tray. Puncture the egg with the rusted pin and crack it open over the photo. Let the gunk cover up the picture as you think about that person getting sick. Sprinkle wood ash over the rotten egg. Let everything sit for a few minutes. Again, concentrate on your intentions as you enjoy the aroma.
Then take the plate outside, and bury the whole mess with the photo facing downwards. Your target will be ill within a week.

On the night of the dark moon, paint the bone completely black (don't miss any bits). Let it dry, and then use the silver needle to scratch 5 small x's in the paint. You might want to get a bone big enough to allow this or it can be frustrating.
Now, you have to put the bone somewhere that the person you are targeting will find it and pick it up. You can't just hand it to them either. They have to find it on their own. It doesn't matter what they think or if they throw it away after. As long as they touch it once, the curse will begin.

Choose your jars so that the little one will fit entirely inside the big one, including when lids are on. A pickle jar and a small jam jar would work fine, for example.
Use a heavy black marker (no pencil or ball point), to draw a pentacle on a piece of paper, then cut it out to fit inside the bottom of the big jar. Place the garlic cloves inside the smaller jar, and tighten the lid. Repeat the first words of the spell:
Moon will wax, moon will wane,
Now I cast the garlic bane.
Set the little jar inside the big one, on the piece of paper. Now mix ash and dirt in water, and pour into the big jar enough to cover the smaller jar. Doesn't have to be completely full, just enough to cover the jar with the garlic. Say:

As the ink washes off the paper into the water, the spell will gain energy and start to work in a few days. Leave the entire thing on your altar for it to keep working. Repeat the words again over the jars to recharge the spell after a month, if necessary.
This is a little witchcraft curse charm bag that you fill up and place as close to your target as possible. Bury it outside their house if you can. If not, possibly hide it in their room, locker, desk or something similar.

If you can't get all 4 herbs, just use what you can. You should have at least 2 of them. For the pentacle, you can use a cheap jewelry charm or carve one out from a round piece of wood. Anything physical will do, just not a drawing on paper.
