It is definitely possible, yes. As Silver said, it is called dream walking. It takes a lot of work to get proficient at it, unless it comes naturally for you. I have experimented with it for a few years and got feedback proving my experiences and have been journaling everything.
Practice, practice, practice. Make journals of your dreams, learn to sense when the individual is drifting off to sleep and follow that energy. Not quite as difficult as some think but not easy either.
